What you should know about Te RAwhiti Community Mental Health Centre
This positions us favourably when approaching the Government to support major new regional capital projects that will enable us to care for a much larger population with more diverse healthcare needs as Auckland continues to grow. Community Integration previously known as Community Health Services Integration or CHSI is a core component of Counties Manukau Health’s Healthy Together strategy and goal of working together to achieve health equity. The transformations will provide more proactive, responsive and integrated community health services to help patients, whaanau and households regain or maintain health, wellbeing and independence. CM Health offers these people more support to better manage their health conditions and keep well.
The establishment of Localities was the first step in the Integrated Care journey. Counties Manukau is made up of four geographical areas where hospital and primary care staff work together to support patients in their community. Counties Manukau Health CM Health provides health and disability services to an estimated 512,00 people who reside in the local authorities of Auckland, Waikato and Hauraki District. Over the next five years, the population of people living in Counties Manukau is growing at 1 2 each year and is changing, both youthful and ageing, and is estimated to reach over 560,00 people by 2020.
Counties Manukau Health was established to plan and fund the provision of personal health, public health and disability support services for the improvement, promotion and protection of the health of our communities. Chief Executive of Counties Manukau District Health Board, Geraint Martin, has welcomed the appointment of Dr Lester Levy to the Board. Dr Levy has significant experience in both health and governance generally, including in previous roles in South Auckland said Geraint Martin.
Geraint Martin also took the time to thank outgoing Chair Dr Lee Mathias and other members of the board for their support over the last few years. CM Health staff and Patient Whaanau Centred Care Consumer council members have responsibility in ensuring that they utilise everyone's expertise to best support our joint ambition of excellent patient, whaanau and staff experience when delivering and receiving care.
